Observation

Licensor learned through the complaint assessment, that the facility failed to report to the appropriate agencies within 24 hours a bruising on a child whose health and safety was being assessed, in addiiton, licensor learned that there was also another bruise that had occurred in the past on the child, and several other old bruises were noticed by the staff member during the ref's visit, Licensor Lauilefue concluded that more likely than not, applicable WAC or RCW was violated.
